Patient Care Coordinator
We believe that patients should have help in navigating the complex maze of cancer treatment options. The Oncology Patient Care Coordinator (PCC) is an Oncology Certified Nurse (OCN) who helps plan and implement a personalized plan of care for each oncology patient. Working closely with the patient and family, physicians, nurses, and all other members of the Cancer Care Team, she works to insure that all the patient’s needs are met. The PCC serves as a resource person for patients and families -- answering questions about treatment, explaining procedures, and supporting patients in decision-making. She assists in the transition from hospital to home or other care facility, and makes follow-up calls to patients following discharge.
